This process is used when a member broker account needs to be removed/cancelled.
1. Click on the HMS icon, select CCM Search.
2. Conduct a search using the broker number. (if not provided, search by main office phone number, address, office name or broker's name)
a. Main office telephone number, click Search icon
b. if no result, search by Main office name, click Search icon
c. if no results, search by Main office address, click Search icon
d. if no results, search by Broker Name, click Search icon
3. Click on the Cancellation Reason tab
a. Click on the down arrow Cancellation Reason Code: select the appropriate reason. ex. Closed Office
b. Click in the Comments field: document requested information. ex. per email from AE Katie..... (your first initial and last name)
c. Effective Date: enter the appropriate cancellation effective date
4. Save
